You are reading a profile of a passionate, committed and stellar case manager. In the past 2 years I have assisted high risk population patients concerning behavior health and chronic pain to maximum potential levels by utilizing the nursing process and applying to evidenced based theories. Thinking outside the box is mandatory for this field, as no two patients are alike, as well as perception of needs are not either. Be it home visit, hospital visit or even office visit this case manager is there for their patients, and not 100, but whatever is required to assist the patient is provided to assist patients to their maximum healthy level with regards to time sensitive and cost containment parameters.
